36-year-old Daimon Michiko is an attractive and independent surgeon. She is not affiliated with the medical department of the university hospital, she moves around different hospitals, while being connected to a suspicious type of 'medical agency'. She observes the working day strictly, does not accept tasks where medical leave is not necessary, and is also interested in the power struggles characteristic of hospitals. Her private life and work experience are shrouded in mystery, but she is a fierce, world-class surgeon with the instincts of a genius. While there are nasty rumors that the medical community left because of financial problems and medical malpractice, no one knows who he really is.

Ryoko Yonekura as Michiko Daimon (36)
Doctor X is a unique character beyond her great ability to operate, she has a double personality. In the hospital, in her work, she is determined, rude, sometimes ruthless in her attempts to get a patient to operate but little by little you realize that all her actions do not come from a selfish desire to prove that she is the best, or to satisfy his own ego, but to the great responsibility that he feels towards the patients, whom he tries to help even though he has to use somewhat peculiar tactics. She is always financially broke and a lousy mahjong player. Despite being unsociable at work, at home she is usually cool, funny and even a bit vulgar.
Ittoku Kishibe as Akira Kambara (65)
He is a retired surgeon, apparently due to malpractice, with a past as mysterious as that of protégé Michiko Daimon. He gives her accommodation in his house, where he also owns a barbershop and in which it seems that it is only a facade for his real job: being a representative of an independent medical agency. He knows very well the dark workings behind hospitals and also the life and work, the strengths and weaknesses of almost all doctors. He is always smiling, he is gentle and, in addition to the bill for his protégé's services, he always gives a melon. Although he always charges Michiko a mysterious debt and leaves her penniless, he often indulges her whims.
Yuki Uchida as Hiromi Jounouchi (33)
Anesthetist physician. Simple but very capable in her work, in addition to having her feet firmly on the ground. She is separated and has a little daughter, whom she adores and is always first in her life. From the beginning, Michiko draws her attention, especially because she is always the protagonist of one of the surgeon's customs after surgery: placing her hand on the patient's chest. She is the only one who managed to meet Michiko as she really is. Her ex-husband is a children's doctor and works in the same hospital as her.
Kei Tanaka as Hikaru Morimoto (28)
He is the least experienced surgeon in the department of surgery. He is a simple boy with a good heart, somewhat innocent but with great ideals. She admires Michiko even though she sometimes misinterprets her or fails to understand her actions. His first meeting with her was at a disco, as Michiko loves to dance alone.
Masanobu Katsumura as Hideki Kaji (48)
He is a surgeon specialized in laparoscopy, a womanizer, divorced, strong-willed, ambitious and very direct. From the beginning, she gets on very badly with Dr. Michiko, prejudging her as insensitive, ambitious and not very sociable. He is always sanctioning her and tends to drag her out of the presence of her superiors when she does not know how to put herself in her place. He accuses her of being a "demon", using her last name with disdain. Despite this, he showed concern for Michiko, knowing how to hide it very well under hostile treatment. Besides Michiko Daimon, this character is the only one who has several TV specials like "Doctor Y".
Kosuke Suzuki as Mamoru Hara (38)
A calm-tempered surgeon, he tolerates Michiko Daimon's character quite well, although at times he is also scandalized by his lack of education and treatment of patients. She usually listens and trusts Michiko's opinions and lets her do it. Sometimes she acts as a referee between her and her partner Kaji when they fight.
Yasunori Danta as Takashi Torii (48)
He is the head of the department of surgery and a candidate for the management of the hospital, he is not a good surgeon but he is an excellent and dedicated scientific researcher, to the point of putting his work before his health. He doesn't have much character, sometimes Michiko gets him out of his mind but usually she comes before him due to his strong character. Despite cheating on his wife with the secretary, he married her out of duty because it was his first surgery and it had left a horrible scar that would prevent him from getting a husband.
Shiro Ito as Ryunosuke Busujima
He is the new director of the university hospital, chosen after the previous director suffered a stroke during surgery he was performing. It was Busujima who had Michiko Daimon's father's clinic closed. He is ambitious and always seeks to harm the protagonist.
