One day, Michiko is visiting a southern island, when she meets passengers who were injured in a luxury cruise accident. One of them is Busujima Ryunosuke, the former director of Teito University Hospital. Michiko quickly treats Busujima. It is then that he realizes something abnormal, he has a huge tumor in his heart, as well as Hepatocellular Carcinoma (liver cancer).

Busujima had to be treated at the National Center for Higher Medical Care, a technologically advanced hospital that the country has staked its prestige on in order to show Japan's medical technology to the world. There is a great commotion regarding this type of surgery at a preoperative conference due to a power struggle between the factions of Teito University Hospital and Saikyo University Hospital, which represent eastern and western Japan. This fight for supremacy has reached a point where even Tendo Yoshihito, the director of the center, fails. The two Universities naturally struggle to be in charge of Busujima's surgery. Then Michiko suddenly appears at the conference and boldly declares, "Let me operate on him. He is my patient." However, she is expelled from the conference. Right after that, the situation undergoes a complete change. Men from both sides hire Michiko and contemplate how to tilt the power game in their favor. In the end, Michiko is employed as the doctor who attends Busujima. But despite everything he went through, Busujima refuses to be operated on by her ...

Ryoko Yonekura as Michiko Daimon (38)
Doctor X is a unique character beyond her great ability to operate, she has a double personality. In the hospital, at her work, she is determined, rude, sometimes ruthless in her attempts to get a patient to operate but little by little you realize that all her actions do not come from a selfish desire to prove that she is the best, or to satisfy his own ego, but to the great responsibility that he feels towards the patients, whom he tries to help even though he has to use somewhat peculiar tactics. She is always financially broke and is a lousy mahjong player (the only one she beats is Kaji). Despite being unsociable at work, at home she is usually cool, funny and even a bit vulgar. She is also fond of horse racing, ping pong, public baths, and tuna fish. This season, for the first time, he will refuse to help a particular patient. It is only known that his father, now deceased, was also a doctor.

Ittoku Kishibe as Akira Kanabara (67)
Almost like a father to Michiko and whom she greatly admires, he is a retired surgeon, apparently due to malpractice, with a past as mysterious as that of his protégé Michiko Daimon. He accommodates her in his home, where his independent medical agency also operates. He knows very well the dark workings behind hospitals and also the life and work, the strengths and weaknesses of almost all doctors. He is always smiling, he is gentle and, in addition to the bill for his protégé's services, he always gives a melon as a gift. Although he always charges Michiko a mysterious debt and leaves her penniless, he often indulges her whims. He has a very spoiled cat named Ben Casey for her pet. In this season the reason for the high amounts of your bills is revealed. He is an excellent mahjong player.

Yuki Uchida as Hiromi Jounouchi (35)
Anesthetist physician. Simple but very capable in her work, in addition to having her feet firmly on the ground. She is separated and has a daughter named Mai, whom she adores and always comes first in her life. From the beginning, Michiko draws her attention, especially because she is always the protagonist of one of the surgeon's customs after surgery: placing her hand on the patient's chest. She became best friends with Michiko and works with her under Akira Kambara as her representative at the independent medical agency. She also likes to play mahjong and is a better player than her friend Michiko.




Masanobu Katsumura as Hideki Kaji (50)
He is a surgeon specialized in laparoscopy, divorced, womanizer and strong, ambitious and very conceited character. Despite the fact that Dr. Daimon very easily drives him out of his mind and he continues to call her "Demon", he generally takes his side and ends up doing the right thing. Like Michiko, she has no interest in positions but has a keen interest in money and fame that puts patients first. He is now subordinate under the orders of Dr. Ebina, forming part of the Eastern Department of Surgery. Despite his ambition and unscrupulousness, he still has some integrity inside him that Dr. X manages to make him come out from time to time. He is a worse Mahjong player than Michiko herself. He is Doctor Y.


Endo Kenichi as Ebina Kei (51)
A surgeon with a "scary" appearance, according to Michiko Daimon, with little character and self-esteem, he still managed to take over the leadership of the Eastern Surgery Department thanks to the fact that he was attributed the multi-organic surgery of little Kurimi that they had performed Drs Daimon, Kondo and Takano at the Totei University Hospital. He is also a bit naive and is usually slow to make decisions and sometimes cannot decide whether or not to do the right thing. He is friends with Kaji and Hara and they are always conspiring to achieve their goals, whether they are honest or not.






Kosuke Suzuki as Mamoru Hara (40)
Former partner of Michiko, Hiromi and Kaji, he reappears this season as a guru of self-improvement success, but quickly falls from grace thanks to Michiko, and ends up hired in the Eastern Department of Surgery, forming a very interesting trio with Kaji. and Ebina. Despite introducing herself to Michiko a hundred times, she never remembers him and calls him by other names. He is a calm-tempered surgeon, he tolerates Michiko Daimon's character quite well, although he is also sometimes scandalized by his lack of education and treatment of patients. He usually listens and trusts Michiko's opinions and lets her do it. Although he is weak in character, he is very sensitive towards patients.


Kitaoji Kinya as Tendo Yoshihito (68) Named "Chancellor", he is in charge of the National Center for Advanced Medical Care, he is an intelligent, cold and calculating man whose ambition is to make Japanese medicine renowned throughout the world and will not hesitate to fire those who do not seem useful to your plans. He knows that Michiko Daimon can help him achieve his goal, but he also knows that she is very dangerous due to her integrity and independence.








Nishida Toshiyuki as Hiruma Shigekatsu (63)
He is the new Director General of Seiko University Hospital, his wish is that his Western Department of Surgery represents the National Center for Advanced Medical Care and he is ready to destroy Michiko along with all those in the East. He does not imagine that things will get too dire for himself later on.









Takahata Atsuko as Shiraki Junko (55) Head of the nurses, known as the "Chamber of Ladies". She is very serious, strict and efficient in her work, she not only controls the nurses, but also does the same with the doctors. She does not like Michiko Daimon's behavior or dress at all, mainly because she considers it mistreatment for her subordinates to make them work after hours. She has a lot of power in the hospital.







Ibu Masato as Dangozaki Noboru (58) Head of the Western Department of Surgery. He hates Ebina and you want to destroy him along with everyone from the East, he does not hesitate to take advantage of her naivety to get away with it.
